Before you register and pay for the online bankruptcy course, please review the following notes carefully:

• Once on the course page, check the system requirements in the Support Section before paying for the session.
• If you need any assistance, you may reach us at (800) 244-1183 during normal business hours Monday through Friday 8:30am to 5:30pm pacific standard time.
• Upon completion, a certified counselor will review your material before issuing your certificate. This will include making sure you completed all of the worksheets and spent the necessary amount of time on the material. You will be asked to mail or fax us a copy of your driver’s license or other valid photo ID to verify your identity. A counselor will contact you by email or phone prior to sending your certificate.

CCNW Privacy Policy/Counseling Disclosure and Fees Agreement CCNW Privacy Policy: Protecting your privacy is of the utmost importance to Consumer Counseling Northwest. Our Privacy Policy describes how Consumer Counseling Northwest (CCNW) handles information that we learn about people who contact us by phone, mail or e-mail, utilize our website, register for an online course, attend one of our classes or presentations, or visit one of our offices. We do not disclose any nonpublic personal information to third parties without a signed release from our client authorizing CCNW to share or to exchange information. Nonpublic personal information includes any personally identifiable financial or demographic information that is not publicly available. CCNW will release nonpublic personal information if legally ordered to do so by local, state or federal courts having jurisdiction over CCNW. CCNW collects nonpublic personal information from information obtained from our client applications; from nonaffiliated third parties concerning the clients' transactions with them; from financial institutions and credit granting institutions reporting on the clients' transactions with them; from collection and recovery groups; and from credit reporting agencies. CCNW may use aggregated information for the purpose of evaluating our services, gathering general demographic characteristics, and creating future community services. No nonpublic personal information that could identify any individual is ever published or available to any non-affiliated third parties through research studies. If you visit a CCNW website to browse, read, order items, apply for online counseling, or download information: • CCNW automatically collects and stores the name of the domain and host from which you access the internet; the internet protocol address of the computer you are using; the browser software you use and your operating system; the date and time you access our sites; and the Internet address of the site from which you linked directly to our sites. CCNW only uses this information as anonymous aggregate research data to determine the number of visitors to different sections of our sites and to help us make our sites more useful. CCNW does not track or record information about individuals. This information is deleted from our system on a regular basis. • CCNW does not use cookies or tracking mechanisms to collect personal identifying information from our sites. If you choose to identify yourself when you call, visit one of our offices, attend one of our classes or seminars, send e-mail, write, or use secure online forms at CCNW: • CCNW collects personally identifying information, such as your name, physical address, e-mail address, phone number, or other information you provide. None of this personally identifying information is shared with third-parties unless CCNW receives written permission from the client that would allow specific information exchanges. CCNW strives to maintain the security of the information you provide: • CCNW uses secure socket layer (SSL) encryption to protect the transmission of the information you submit to us when you use secure online forms. This information is stored securely. All employees have signed confidentiality agreements that safeguard your privacy and only specific employees are authorized to access your nonpublic personal information while serving your interests or performing quality control reviews. E-mail that you send to CCNW is not necessarily secure against interception. If your e-mail communication contains sensitive private information, such as account numbers or your Social Security number, please communicate that information to CCNW via secure online forms, written mail, by land-line, hard-wired telephone or in person. • The CCNW website provides links to websites maintained by other agencies and organizations. Once you access another group's website, you are subject to the privacy policy of that site. Counseling Disclosure and Fees: Consumer Counseling Northwest maintains the highest standards of quality. Our counselors are NFCC certified credit counselors who have passed seven examinations and are also certified by the Consumer Data Industry Association on the Fair Credit Reporting Act. CCNW is HUD approved to perform housing counseling, both pre and post purchase, mortgage delinquency and reverse mortgage counseling. CCNW is accredited by the Council on Accreditation of Children and Family Services. CCNW receives funding from creditor contributions, grants, special programs and contracts, and monthly client servicing fees.. Use of a credit counseling agency to pay off debts may be seen adversely on your credit report. Servicing fees for debt management repayment plans varies. Cost for Bankruptcy counseling is $50 per person or $75 per couple. Costs may be paid by cashier’s check, debit card or money order at the time of service. CCNW will provide services without regard to a client’s ability to pay and will not withhold a certificate of counseling because of an inability to pay; therefore hardship waivers are available. • I authorize Consumer Counseling Northwest (CCNW) to counsel and instruct me on credit and money management. • I understand that the decision to implement any suggestions made by CCNW is entirely up to me. • I have read and understand the privacy statement. I authorize CCNW to release any and all non-public information it obtains about me to my creditors and any third parties necessary to resolve any matters discussed during my counseling session. • If requested, I authorize CCNW to release a copy of my Bankruptcy Certificate to my attorney and will provide the attorney’s name, address and fax # if needed. • I further release and authorize all of my creditors to provide non-public information about me to CCNW. • I agree to hold CCNW, its officers, directors, agents, employees and volunteer harmless from any claim, suit, action or demand of any kind from my creditors, others or myself.
